3D Maxillofacial Imaging Centers, PLC is dedicated to fast and efficient acquisition of your 3-D image and the transfer of this information to the referring dentist. Your scheduled visit will take approximately 15 minutes of your time from start to finish. Prior to your appointment, your referring doctor would have sent our office a prescription form of what he/she would like to see. Please bring your own referral form with you for cross-reference. When you arrive at our office, you will provide us with some relevant personal data which we enter into our computer system (Note: All patient information will be private and confidential as per the Health Insurance Portability and Accountability Act regulations (HIPAA, 1996).
